These creatures can be found in the Underdark, specifically in a hidden area called The Festering Cove.
The Festering Cove can be accessed by jumping across some mushrooms near the Arcane Tower and the Decrepit Village in the Underdark.

The Kuo-Toas worship a deity named BOOOAL.
They originated during the era of ancient dragons, serving as loyal subjects and advisors to these mighty creatures.
How To Reach Fish People?
The Festering Cove, tucked away in the Underdark of Baldur’s Gate 3, remains hidden and remote.
To access it, locate the entrance near the Arcane Tower and the Decrepit Village in the Torchstalk field.

Leap two mushrooms on the cliff’s edge to reach a Cragged Rock.
Further, descend the cragged rock to step into The Festering Cove.
What To Do In The Festering Cove?
If you reach The Festering Cove, you can see the BG3 fish people worshipping the BOOOAL.
Go and talk with the priest, and you will get various dialogue options.
Here is what you can get with various dialogue options.
1. Blessing Of The God
- Choose “‘BOOOAL’? Bhaal is the god of murder” to get the blessing of the BOOOAL.

- Choose “Wait! If it’s blood you want, I can make another offer.”

- You must kill your companion if you choose the first option and give a blood sacrifice.

- As a blessing, BOOOAL will give you BOOOAL’S Benedectiom Buff, which gives additional damage to the bleeding creatures.

2. Sickle Of BOOOAL
- Choose the option “Noone Offering.”

- Choose “Wait! If it’s blood you want, I can make another offer.”

- Choose “I have only one life. But I can kill many more in your name” for the sickle.

- The BOOOAL will give you the Sickle of BOOOAL.

3. Become A BG3 Fish People God
- Choose the other options rather than the 2nd or 3rd option.

- Choose “Seriously – BOOAL? Whatever this thing is, he didn’t even feed you a good life.”

- Fight with the BOOOAL and defeat him.

- The fish people will make you their god and ask for the first commandment.
